According to the YouTube description the game is being played between Pogon Lwówek and Czarni Wróblewo. With the game tied at 1-1 in the 88th minute Czarni Wróblewo quickly counter-attack with their number 10 breaking clear of the defence to go one on one with the opposition keeper.

He's not selfish though and slides it across to his much better placed teammate. With the ball on the edge of the six yard box and no keeper in front of him, he does what you would probably consider to be nearly impossible and manages to hit it against the crossbar.
The ball rebounds out to his teammate who puts the ball into the back of the net is called offside by the ref.
